Telemonitoring cardiothoracic post-surgical patients

We are implementing a telemonitoring program at the Cardiothoracic Surgery Department of Hospital de Santa Marta to follow patients during their first postoperative month after discharge. Patients use a smartphone, an electronic scale, a sphygmomanometer and a smart watch to daily measure weight, blood pressure, heart rate, rhythm irregularities and step count.

Patients use a mobile app to answer a self-reported outcomes and take a picture of their operative wound and send it to the clinical team at the hospital. We are co-designing comprehensive daily reports with surgeons and nurses for an efficient follow-up of patients at home.
